Step 2: Complete the official Personal Accident Claim Form provided by your insurer.

Step 3: Compile the necessary original supporting documents (see below) and submit them along with the completed claim form to your insurer via post or their designated online portal.

The documents required depend on the nature of your claim:

  • For Medical Expenses Reimbursement:
  • Itemized medical invoices and receipts.
  • A doctor's report stating the diagnosis and confirming the treatment was for injuries sustained in the accident.
  • For a Death Claim:
  • Original Death Certificate.
  • Police report (if applicable).
  • Autopsy report (if required by the insurer).
  • Proof of relationship between the deceased and the claimant (e.g., birth certificate, marriage certificate).
  • For Permanent Disability:
  • A medical and/or specialist's report detailing the nature and extent of the permanent disability, often requiring a specific assessment based on the policy's definitions.
  • For Temporary Total Disability (Loss of Income):

Medical certificates confirming you were unfit for work and stating the exact period of medical leave.
